HMC began work in 2013 with the Affordable Care Act to mobilize stakeholders and build community to address the changes in health care policy and to ensure equitable access to quality assistance.

HMC was awarded a cooperative agreement in 2021 from the Centers for Medicare & Medicaid Services to implement for the State of New Hampshire, community-based health insurance assistance navigator program.

 

In 2025, the Commonwealth of Pennsylvania selected HMC to be the Lead Contractor to subcontract and manage a network of health insurance assisters through its state based marketplace, Pennie.

 

Guided by the principles of Asset-Based Community Development and Cultural Agility, we work to increase access to high quality health insurance assistance.
